[STDS-802-16] Rejected contributions for P802.16Rev2/D2

Regarding the contributions submitted towards the P802.16Rev2 project in Letter Ballot 26a, a number of contributions will be rejected unless the technical issues with them are corrected.  The problematic contributions have been categorized and listed below:
