802.16 Session #5 Registration Form

January 10-14, 2000

The registration fee is $140 US if you register by the advance registration deadline of noon (Dallas time) on 07 January 2000 and $165 US otherwise. Please register early so we can arrange sufficient meeting space. You need not pay the fee until you arrive at the meeting. The only acceptable forms of payment will be a check or money order in US funds payable to "Raytheon". If you do not pre-register, you may register on-site.

Please register me for 802.16 Session #5. I intend to attend and will notify the organizers immediately if I my plans change.
I plan to attend the complimentary dinner hosted by Crosspan on Wednesday evening, 12 January 2000.
