802.16 Session #5 Email Registration Form

The registration fee is $140 US IF you register by the advance registration
deadline of 31 December 1999 and $165 US otherwise. You need not pay the fee
until you arrive at the meeting. The only acceptable forms of payment will be a
check or money order in US funds payable to "Raytheon".
