I did not see a fully worked proposal for CCM or GCM that showed
how the key/counter uniqueness was established.  SIV solves this issue.

The algorithms (SIV versus CCM or GCM) are not direct replacements.
SIV does not require key/counter uniqueness.  This is not to
say that additional mechanisms and text could be added to meet
this aditional CCM/GCM requirements.
