stds-802-16: Outline of the 802.16.1 Air Interface Standard (Draft, 99/09/23)

I've posted an important new document:

	Outline of the 802.16.1 Air Interface Standard (Draft, 99/09/23)

I created this outline by merging outlines submitted by the PHY and MAC groups.

I posted it in Word format so that the groups can edit easily. However, the groups should edit only their own portion of the outline so that we minimize version control problems.
