stds-802-16: new contribution and presentation templates



Our current Calls for Contributions specify the use of Revision 7 of 
the 802.16 Document Submission Template.

I have created and posted this template: 
<http://ieee802.org/16/docs/802_16_template.doc>.


I have also updated the "Cover Sheet for Presentation to IEEE 802.16 
Broadband Wireless Access Working Group" from Rev. 0 to Rev. 1. The 
new version is at:
<http://ieee802.org/16/docs/802_16_present.ppt>.

All of the changes are editorial.
