stds-802-16: Document Submission Template in FrameMaker

We now have an 802.16 Document Submission Template in FrameMaker:

Thanks to Stanley Wang for taking the initiative to create this. I 
did a little editing, including importing the IEEE standard formats.

By the way, I have updated all the templates slightly and increased 
the version numbers to 8.21 (from 8.2).
