stds-802-16: NEW new revision of TG1 draft for review



I found I'd made a editing mistake in D3d2 that can make reading 
pretty difficult. Therefore, I've just posted a new version. The PDF 
is identical, except that I've added "annotations" on top of it to 
show the changes.

The mistake was an accidental quasi-global change of "fixed-length" 
to "variable-length". The annotations change "variable-length" back 
to "fixed-length" in the following six places:

Page 53, Line 18
Page 63, Line 18
Page 108 (all four instances in 6.2.3.4.1)
