[STDS-802-16] release of P802.16-REVd/D4

P802.16d/D4 is now available:

as is the comment database used to derive it:

and a version showing the markups as compared to D3:

I have submitted the last one for 15-day Sponsor Ballot
recirculation, which I understand will open by tomorrow.

On another topic: P802.16e/D2 should be available for recirc early next week.
