[STDS-802-16] version of 802.16j as submitted to NesCom

Folks,

In order to get the 802.16j PAR onto the 29 March NesCom agenda, I submitted it to them before the deadline. This required me to make some editorial changes, mainly because the PAR form has changed. I propose that we adopt these changes. Here is the version I submitted:
