In the original COR1 document (D5) there was a fix to this section, apparently it did not make it to 802.16e-2005 version (another item which was missed by the reviewers of this section....). Anyway, try to give a look at P80216_Cor1_D5 draft (should be available at the following link:

I am building 802.16e baseband model. I have tried to test each module one by one and I have found that the example given in pg 619 of 802.16-2004 is probably wrong ?  I have tried  Ncpc = number coded bits per subcarrier = QPSK = 2, while Ncbps = total bits to be transmitted after CC encoding & punctuing = 24 x 8 = 192 bits. Using Ncpc = 2 and Ncbps = 192 does not give me the correct results, but interestingly, using Ncpc = 16QAM = 4 and Ncbps = 192 give me the correct answer. Is the example sin 802.16-2004 wrong ? Is there any corrected version ? I have not been able to find the correction in latest document 802.16-2005. Would anyone be able to give me some clarification ?
