[STDS-802-16] Rejected contributions for P802.16Rev2/D7, Sponsor Ballot Recirc

Regarding the contributions submitted towards the P802.16Rev2 project in Sponsor Ballot Recirc, a number of contributions will be rejected unless the formatting issues with them are corrected.  

The problematic contributions have been categorized and listed below:
