The bit mapper is found in 55.3.2.2.19 and 20.

The demapper is left as an exercise to the implementer using the information described in the mapper.

If it is not clear how to do this, I suggest you consult a person knowledgeable in DSP coding.

We are working on 10G BASE-T and found that there is no information regarding bit demapper   4D PAM16 to DSQ128.

Can you please let us know where to find more information on this?
