stds-802-16: New Template for IEEE 802.16 documents

I have created a new template for IEEE 802.16 documents. The new template,
in Word format, is available at

Item IEEE 802.16-99/01r2 ("Template for IEEE 802.16 documents, Rev. 2")

documents the new version.

In the new template, four line items have been added to the cover page:

*Date Submitted:
This documents the date of submission, which may differ from the release date.

*Re:
This asks the submitter to specify either the document number to be revised
or the Call for Contributions (and relevant item therein) to which the
document responds. This should help focus submissions directly on the topic
specified in the Call for Contributions. If the document is non-responsive
on this point, it may be refused or consigned to the "General
Contributions" area.

*Purpose
This section is made a separate line item, rather than joined with
"ABSTRACT" as in the original template. It requires the submitter to
clearly specify the intent and the actions requested.

*Release
This provides notice that the contribution is not confidential.

Also, the instructions for using the template and submitting the
contribution have been clarified and simplified.
