No doubt the wording can be improved. The three address format was
intended as a way to mean the 2nd or 3rd entry in the table in 8.3.2.1
which entries have the specified address field equalities. It wasn't
meant to include the 1st entry of the table in 8.3.2.1. Because of the
constraints from that table, I think it is implied that FromDS must be
on in transmissions from an AP and ToDS must be on in transmissions
from a STA in an infrastructure BSS.

As above, "3-address frame format" is means to be the two cases where
FromDS is not equal to ToDS. This page 56 statement seems so general
that I think it implies that usual conditions apply for using these
various formats.
