[STDS-802-16] Clarification on OFDMA Uplink symbol structure

I have a few questions regarding the 802.16-REVd/D4 spec, and I am
hoping that the email reflector is a good way to resolve them.  Here is
one question:

For the OFDMA PHY, there are three possibilities for the uplink
symbol/subchannel structure for PUSC -- the one essentially defined in
8.4.6.2.1, the "additional optional symbol structure" defined in
8.4.6.2.4, and the adjacent subcarrier permutation defined in 8.4.6.3.
How does the BS indicate which of the first two it is using (when not
using the third)?
