My observation of Table 9.4, Rows 5 and 6 is that they use parameters only
valid for dual-queue implementations--in particular STQDepth,
STQLowThreshold, and STQHighThreshold.

Should the ORed pieces in the conditions of these rows also be related to
dual or single queue configuration? I attempted to locate a relavant
comment/comment resolution, but don't think I found one.
