[802.3ae] 64b/66b block lock state machine

Title: 64b/66b block lock state machine

Hi,

Can someone answer a few questions for me on the lock state machine in 802.3ae/D3.0, Figure 49-14?

1.  Where/when is test_sh set to TRUE? 
     It's set to FALSE when TEST_SH state is entered, but I don't see anywhere in 802.3ae/D3.0 stating when this variable should be set to TRUE.

2.  What is the next state if the current state is RESET_CNT and test_sh=FALSE?

3.  What is the next state if the current state is VALID_SH and test_sh=FALSE and sh_cnt < 64?

4.  What is the next state if the current state is INVALID_SH and the test_sh=FALSE and sh_cnt < 64?
