Michele,

Both of the mechanisms that you describe are present.   As you indicate, receipt of an "on the fly" DSA forces the Base Station to interact with the Management and Provisioning System to receive approval for the new SF and a unique SFId.  

To avoid that overhead, providers are likely to do what they do in Cable networks - ie. set up pre-provisioned Service Flows with no classifiers and an initially null Active QoS Parameter set, so that the SS or Network Management can subsequently perform DSC.
