The answer imho is "yes" and "no".
When receiving the DIUC 0x00, an SS must start measuring if it's not already doing so.
It is however possible to respond to DIUC 0x00 with an unsolicited REP-RSP if the data is already available, rather than wait for a REP-REQ to respond to. The choice in SS implementation is hence to continuously measure, or only measure after being ordered to by the BS.
Regarding the second item, note that it says "for each channel". That is, if the report is issued with measurements from channel 'x', then all counters for channel 'x' should be reset. However, any counters running for channels other than channel 'x' should not be reset.
Even if only RSSI is reported, the CINR one must be reset for that channel as well. The SS may however volunteer the CINR data in addition to the RSSI data even if the BS only requested RSSI.
Implementations whereby the BS requests the RSSI and almost immediately following the CINR are hence going to result in inaccurate results for CINR. In practice, if the BS operates on both values, it should request them simultaneously.

I think a little clarification is needed on 2 points regarding the channel
measurement report mechanism.
In section 8.3.5.2.3, it is stated : "An extended IE with an extended DIUC
value of 0x00 is issued by the BS to request a channel measurement report
(see 6.3.2.3.33". My interpretation is that it requests a channel
measurement, but not a report. A report would be requested by a REP-REQ MAC
message. Is that correct?
Second related point, in section 6.3.15.6, it is written "Upon receiving a
REP-REQ the SS shall reply with a REP-RSP message and reset its measurement
counters for each channel on which it reported". My interpretation is that
it should reset all counters, and not only those it has reported. For
instance, if it reported on RSSI only, should it reset its CINR counter?
